As a case manager, you will work closely with patients and their families to develop a long-term healthcare plan that considers their preferences and goals.

Typically, case managers work with patients diagnosed with one type of disease like HIV/AIDs or cancer, or they work with patients in a specific age group such as children or the elderly.
